2. Completion of this form indicates my interest to subscribe to service and is a necessary step to determining the viability of a new broadband network.

3. I hereby agree to receive future communications, marketing material and construction updates by submission of this form.

4. I understand that before construction & installation of a broadband fiber network a successful pre-subscription deposit campaign and service contract may be necessary to fulfilling a commitment of services and/or completion installation of the network to my location.
